DEPARTURE PORT: Port of Los Angeles, CA – San Pedro, World Cruise Terminal. Airfare and transportation to and from your port of call are not included in the Cruise for Life Recognition Program.

ACCOMMODATIONS:

* Each ticket is $475.00. This ticket provides you with one twin bed within an Ocean View Stateroom.

GUESTS and/or PRIVATE ROOM:

* If you would like to bring a guest or purchase a private room, there will be an additional charge of $475.00*.

PLEASE NOTE: Each stateroom is double occupancy. If you are not paying for a guest to accompany you, or for a private room, a roommate will be chosen for you.

IDENTIFICATION: You must have a valid passport in order to board the ship.

RSVP NO LATER THAN JULY 16, 2008:

* You must contact Corporate Events no later than July 16 to reserve your stateroom and/or add guests to your reservation.
* Please RSVP to the email address listed in your back office Bulletin Board. Your confirmation must include the following information:

* NAME: Your full name as it appears on your passport.
* GUEST INFORMATION (if appropriate): Guest’s full name as it appears on his/her passport and credit card information used for payment*.
* CREDIT CARD: A valid credit card will be kept on file. IMPORTANT: If, for any reason, you do not board the ship, the card on file will be charged for the cost of the cruise.*
